ChamberChoice is making this offering even easier for its members by providing a program that includes three different benefit levels. If you choose to offer HM Care Advantage, communication materials that explain the coverage will be made available to you for distribution to your employees. The materials contain a toll-free phone number to the HM Care Advantage Call Center so your employees can speak with a benefit specialist who can address additional questions and/or help them enroll.
HM Care Advantage is an HM Life Insurance Company product administered by Key Benefit Administrators (KBA). Based on the plan selected, Medical and Vision coverages are underwritten by HM Life Insurance Company, Pittsburgh, PA, under policy form series HM905 or similar. For other insured products when available: Dental and Outpatient Prescription Drug coverages are underwritten by Fidelity Security Life Insurance Company, Kansas City, MO, under policy form series M-9037 and M-9031/M-9022.
